# How to Create a Form and Add Email/SMS Verification To It

To create a form with Contact Form 7 and add 2 step verification to it, go to Contact > Add New from the WordPress dashboard.

Here, when you add the phone number field and email field, just insert the keyword verify next to the form label. Adding this keyword enables verification for that field.

For example, if you only want SMS verification, add the keyword verify next to the label of phone number and if you want email verification add the keyword next to the label of email field. To add both verifications, add the keyword verify to both form fields.
